GBA Insurance Trust Medical Plans
The GBA Insurance Trust has partnered with Anthem to offer participants a managed care program that includes a statewide network of hospitals and physicians. In addition, the Trust provides a prescription drug program that includes virtually all Georgia pharmacies. The Summary of Benefits and Coverage for each plan option summarizes the plan cost sharing provisions. The SPD/Certificate Booklets provide important information about the terms and conditions that apply to the plan.
